We are an out-of-network provider for most health insurances. Reimbursement is determined by your insurance company provider.
Dr. Christiansen provides a receipt with the necessary information required for reimbursement from your insurance company. Please be sure to include a copy of this receipt when filing your reimbursement claim form.
